Read document; fill in your name, company name, sign your name, fill in your title and date. Then, make a copy for yourself and send the original document to Park Place Gardens Nursery. We will fill in the invoice number and give you notification of which items apply to the Waiver.

Suitable Plant Materials Waiver

I, __________________________________________ with___________________________________________ understand that the items designated by Park Place Gardens Nursery(PPGN) on Invoice Number ___________ are not suitable and not recommended by the PPGN, for the area I / we will be planting. It is therefore understood, that no warranty is given or expected over and above the warranty statement in section six ( 6 ) of Park Place Gardens Nursery "Terms and Conditions."
